re #8: I agree that it's surely standard in rhetorical exclamations. In fact, just 'Well, did you ever!', without any other verb, is a fairly fixed phrase, though it's now dated to obsolete.
I'm also skeptical that 'Did you ever ...' isn't used in a few other contexts in BE, or dialects thereof, as well.
Or at least, it seems to have been in the past. Another nursery song (cf. #8a):
Did you ever see a lassie, a lassie, a lassie,
did you ever see a lassie go this way and that?
However, MikeE's other two examples (#8b, c), the popular songs, could easily both have been originally AE. Isn't the first one 'What a swell party this is'?
Wonder if it could be one of those things like 'gotten' that's actually older in the language, and survived only in AE?
