    Mit "insurance" ist wahrscheinlich Krankenversicherung gemeint.

    OK - first of all the English is lousy.

    I'll explain what they are talking about.
    This is about drug coverage under a private health insurance plan.
    Brand name drugs are generally very expensive. If there is an equivalent, patented but "generic" brand drug, the insurance company will have you substitute for the cheaper drug. i.e., they will automatically only cover (pay for) the cheaper of the two. If you insist on the name brand, you will pay for the difference between the generic brand and the name brand out of pocket.
