Even ten years later, I don't think "internal specialist" is often used in this context, in either AE or BE, Bama Torsten's "internist" sounds much better, or "specialist in internal medicine".
=> a primary care internist working in a joint practice
=> specialist in internal medicine working in a primary care joint practice
=> specialist in internal medicine, working as a primary care physician in a joint practice
