I would still think that "sick note" is worthy of inclusion, with [BE] flag, as this is probably the most idiomatic.
Legally, a doctor is required to issue a statement of incapacity for work due to illness if the illness lasts for more than 7 days (including Sunday).
form Med 3 is the standard sick note
form Med 5 is used of the patient is seen by a different doctor, or requests a note more than a day after being seen
